Please bring a valid, state issued ID (Driver’s License or State ID with name, photo, and current address) or one photo ID and one document with name and current address. The signature of a parent or guardian is required for applicants under 16.

Once we review your information and create your account, we will mail your card to your Lincolnwood address.

Illinois Residents

Cardholders of libraries within Illinois can register their card for reciprocal borrowing privileges. Please bring your ‘home’ library card and valid state issued ID (Driver’s License or State ID with name, photo, and current address) or one photo ID and one document with name and current address to the Circulation Desk. Cards from libraries outside the Cooperative Computer Services (CCS) Consortium will need to be re-registered every two years.
